Healthy Weight Action Plan

I heard this talked about a couple times over the weekend on CP24 and thought I'd check it out. It seems pretty cool.

The Heart & Stroke Healthy Weight Action Plan (HW Plan) is an online tool developed by experts and based on research studies. The HW Plan is a free 12-week, step-by-step program that will support you in achieving healthy eating habits and a healthy weight—for life. It is not a fad diet nor do you have to purchase anything.

To start, you complete a Health Assessment. This will allow you to find out if the HW Plan is an appropriate program for you. The assessment also allows them to tailor the program to your needs.

The Weekly Lessons

Lesson 1 - Self Monitoring
In this lesson, we'll focus on why it's important to monitor what you are eating and/or how active you are.

Lesson 2 - Goal Settings
In this lesson, you will receive feedback on your week of self monitoring that will help you determine your goals. You will also receive a tailored recommendation for how much to eat every day. Finally, you will learn about the importance of setting goals, and how to input your goals into the Goals & Tasks section.

Lesson 3 - Tracking Your Behaviour
In this lesson, you will learn why it is so important to consistently track your progress and what tracking options are available to you within the HW Plan.

Lesson 4 - Planning for Challenges
In this lesson, you will identify challenges you may face when pursuing your goals and create a plan of action to overcome those challenges.

Lesson 5 - Social Support
In this lesson, you will learn about the different forms social support can take, understand how to optimize your social support system, and identify who will be part of your support team.

Lesson 6 - Setbacks and Glitches
In this lesson, you will learn about setbacks and how to manage them effectively as well as how to plan ahead to develop a more flexible plan.

Lesson 7 - Reward System
In this lesson, we will take a look at the importance of rewards, the types available, and how to create meaningful rewards within your own plan of behaviour change.

Lesson 8 - Behaviour Contract
In this lesson, you will learn the benefits of drawing up a personal behaviour contract and have the opportunity to review your own contracts.

Lesson 9 - Time management
In this lesson, you will learn how to overcome the challenge of lack of time as a barrier to activity and healthy eating.

Lesson 10 - Stress management
In this lesson, you will learn how stress can impact your ability to achieve or maintain a healthy weight. You will also get tips on how to manage stress.

Lesson 11 - Plateaus
In this lesson, you will learn about how you can overcome the common and frustrating challenge of reaching a weight loss plateau.

Lesson 12 - A new beginning
In this lesson, you will learn about how you can sustain your healthy eating and physical activity choices to continue achieving a new and healthier lifestyle.
